3 Easy Steps to set up EFT

1. Download and fill out an EFT form using this link, Download EFT Form

2. Write a check addressed to BLF, using the account you want the EFT to be drawn from.

3. Send the form and the check to BLF, PO Box 629, Wheaton, IL 60187.
